TRICARE East Certificate of Medical Necessity (DME)

Humana Military uses this one-page Certificate of Medical Necessity to support TRICARE East claims for durable medical equipment, including CPAP devices, breast pumps and capped rental items. The ordering provider certifies the diagnosis codes, HCPCS or CPT codes, quantity and length of need, and the DME supplier sends it with the claim so Humana Military can process it.

Who completes it

The ordering provider completes and signs it with the sponsor ID, beneficiary details, provider name and TIN, diagnosis codes, item lines and start and end dates of need; the DME supplier attaches it to the claim.

Submitting and payment

How to submit it
Send it with the DME claim, by fax to the claims number on the form or through Humana Military provider self-service, adding the claim's internal control number (ICN) if available.
Who pays
Capped rental items are covered for a 15-month period, and if the CMN does not cover the entire rental, another CMN is required before claims after its end date can be processed.

Good to know

  • For a CPAP device, the form asks for the beneficiary's oxygen levels so the device can be properly set.
  • For a breast pump or breastfeeding supplies, enter the gestational age in weeks and the corresponding diagnosis code.
  • Each item line lists the HCPCS or CPT code, description, quantity and notes, followed by the length of need and start and end dates.
  • Current version XPBB0425-A. Humana Military lists it with its claims forms, alongside the DD 2642 claim form and the reconsideration coversheet.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long does a TRICARE CMN cover a capped rental?

Capped rental items are covered for a 15-month period. If the CMN's start and end dates do not cover the entire rental period, another CMN will be required to process claims after the end date, so the length of need should reflect the full rental.

What extra details does the TRICARE CMN ask for CPAP and breast pumps?

If the DME item is a CPAP device, the form asks for the beneficiary's oxygen levels so the device can be properly set. If it is a breast pump or breastfeeding supplies, it asks for the gestational age in weeks and the corresponding diagnosis code.

Who signs the Humana Military CMN?

The ordering provider signs and dates it after entering the diagnosis codes, item codes, quantity, length of need and start and end dates. The DME supplier then sends it with the claim, including the claim's internal control number when one is available.
