TRICARE DSM-5 Autism Diagnostic Checklist (ACD)

TRICARE's Autism Care Demonstration requires the DSM criteria behind an autism spectrum disorder diagnosis to be documented on this two-page checklist before ABA can be authorized. The diagnosing provider marks each criterion as present or not, rates severity in both domains, and notes intellectual or language impairment and comorbidities. It travels with the Referral for Applied Behavior Analysis to Humana Military.

Who completes it

The ASD-diagnosing provider completes the whole checklist and adds their name, credentials, signature, the date of diagnosis and any known comorbid conditions.

Submitting and payment

How to submit it
Civilian providers and military hospitals or clinics submit it through Humana Military provider self-service or by fax.

Good to know

  • Criterion A needs all three social communication deficits; criterion B needs at least two of four restricted, repetitive behavior patterns, currently or by history.
  • Each domain gets a severity rating of requires support, substantial support or very substantial support, as described on page 52 of the DSM-5.
  • A well-established DSM-IV diagnosis of autistic disorder, Asperger's disorder or PDD-NOS is reclassified by checking the box and completing the checklist.
  • The PDF has no fillable fields. Current version XPLR1224-A; Humana Military lists it as the ACD DSM-5 diagnostic checklist.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the DSM-5 checklist required for a TRICARE ABA referral?

Yes. The Referral for Applied Behavior Analysis states that the DSM criteria must be documented in the DSM-5 diagnostic checklist. The checklist and a validated assessment tool with results support the definitive ASD diagnosis that must be approved before ABA starts.

Can an older DSM-IV autism diagnosis be used for TRICARE ABA?

The checklist has a box for individuals with a well-established DSM-IV diagnosis of autistic disorder, Asperger's disorder or PDD-NOS. The provider checks it and completes the checklist to reclassify the earlier diagnosis as autism spectrum disorder under DSM-5 criteria.

What must the diagnosing provider record on page 2?

Whether ASD criteria are met, whether it is with or without intellectual impairment and with or without language impairment, known comorbid conditions (medical, genetic, neurodevelopmental, behavioral health or other), the date of diagnosis, and the provider's name, credentials, signature and date.
