TRICARE Brand over Generic Prior Authorization Form

TRICARE's pharmacy benefit requires substitution of FDA A-rated generics, so a prescriber who wants a TRICARE patient to receive the brand-name drug completes this one-page Express Scripts request. It asks whether the patient tried the generic and what happened, or why the generic cannot be used. An approval has no expiration date and covers prescriptions filled through the TRICARE pharmacy program.

Who completes it

The prescriber completes and signs all three steps: patient and physician information, the clinical questions about the generic, and the certification signature.

Submitting and payment

How to submit it
The prescriber calls or faxes Express Scripts, or the patient attaches the completed form to the prescription and mails it, or emails the form alone, to the TRICARE pharmacy prior authorization contacts printed on the form.
Who pays
TRICARE notes that beneficiaries generally pay more out of pocket for brand-name drugs than for generics.

Good to know

  • The form cites 32 CFR 199.21(j)(2): TRICARE generally requires substitution of A-rated generics, and the policy is waived when there is clinical justification for the brand.
  • If the patient tried the generic, Question 2 asks about their experience with it; if not, Question 3 asks for patient-specific clinical justification for why the A-rated generic cannot be used.
  • The latest revision is dated 10 August 2011. The PDF's internal title still reads PDE-5 Prior Authorization Request Form, but the printed heading is Brand over Generic.
  • TRICARE dispenses a generic substitute unless the generic is unavailable, Express Scripts approves the brand, or TRICARE finds the brand the better value.

Frequently asked questions

How long does a TRICARE brand over generic approval last?

The form states that this prior authorization has no expiration date. It applies to prescriptions filled through the Department of Defense TRICARE pharmacy program, which Express Scripts administers for mail order and retail fills.

What must the prescriber explain on the brand over generic form?

Whether the patient has tried the generic. If yes, describe the patient's experience with it; if no, give patient-specific clinical justification for why the A-rated generic cannot be used. The prescriber then certifies the answers are correct and signs and dates the form.

Can the patient send in the brand over generic form?

Yes. The prescriber may call or fax Express Scripts, but the patient can also attach the completed form to the prescription and mail it to Express Scripts, or email the form alone. The prescriber's signature is still required.
