TRICARE East Referral for Applied Behavior Analysis

Before a child can start applied behavior analysis under TRICARE's Comprehensive Autism Care Demonstration (ACD) in the East Region, Humana Military needs a complete referral with a definitive autism spectrum disorder diagnosis. This form captures the diagnosis with symptom severity, comorbidities, the initial diagnosis date, ECHO registration and the assessment services requested. The referral must be approved before ABA begins.

Who completes it

An approved ASD-diagnosing provider or specialized ASD-diagnosing provider, such as the primary care manager, a developmental pediatrician, psychologist or psychiatrist, completes the whole form and includes a validated assessment tool with its results.

Submitting and payment

How to submit it
Upload it with the referral or authorization request in Humana Military's provider self-service portal at HumanaMilitary.com; fax it only if online submission is not possible.
Who pays
The referral requests CPT 97151, the behavior identification assessment and treatment plan, with one unit for each completed outcome measure (PDDBI, PSI/SIPA, Vineland-3 and SRS-2).

Good to know

  • If ASD was first diagnosed at age eight or older, an evaluation by a specialized ASD-diagnosing provider is required.
  • The DSM-5 criteria must be documented on the separate ACD DSM-5 diagnostic checklist, which goes with the referral.
  • For a dependent of a service member, the form asks whether the child is registered in the Extended Care Health Option (ECHO).
  • Full guidance is in TRICARE Operations Manual Chapter 18, Section 3, Paragraph 4. The current file dates from April 2025.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Who can refer a TRICARE beneficiary for ABA?

An approved ASD-diagnosing provider or a specialized ASD-diagnosing provider, such as a primary care manager, developmental pediatrician, psychologist or psychiatrist. The referral must give a definitive ASD diagnosis with symptom severity and a validated assessment tool with results, and a specialized provider is required if the child was first diagnosed at age eight or older.

What must accompany a TRICARE ABA referral?

The ACD DSM-5 diagnostic checklist documenting the DSM criteria, plus the validated assessment tool and its results. The referral itself lists the ASD diagnosis and severity level, medical and psychological comorbidities, the initial diagnosis date and the CPT 97151 units requested.

Can ABA start while the TRICARE referral is pending?

No. TRICARE requires the complete referral to be submitted and approved before ABA services begin. After approval, the ABA supervisor submits the initial assessment and treatment plan, which also must reach Humana Military before treatment starts.
