Michigan MDHHS-6200 Home Help Medical Certification

Michigan's Home Help program pays for personal care through Medicaid, and a medical needs certification is a prerequisite. The MDHHS-6200, new in September 2025, replaces the DHS-54A for this purpose. A Medicaid-enrolled provider certifies the person's diagnoses, whether the condition is chronic, mobility and equipment needs, and which ADL, IADL and complex care tasks need hands-on assistance.

Who completes it

A Medicaid-enrolled physician (M.D. or D.O.), physician assistant, nurse practitioner, occupational therapist, physical therapist or clinical nurse specialist completes Sections 3 and 4, including NPI and signature. The patient signs the release in Section 1, and Section 2 is for departmental use.

Submitting and payment

How to submit it
Return it to the Adult Services worker, using the fax number the department enters in Section 2.
