Michigan DHS-49-D Psychiatric Examination Report

When a Michigan client claims disability based on a mental health condition, the Disability Determination Service (DDS) relies on the DHS-49-D. A psychiatrist or fully licensed psychologist reports mental status, diagnoses and prognosis, and DDS uses it with the DHS-49-E functional assessment to decide State Disability Assistance, Family Independence Program deferral or Medicaid disability.

Who completes it

A psychiatrist (M.D. or D.O.) or fully licensed psychologist completes the whole report. The client completes the separate DHS-49-F questionnaire and signs the DHS-1555 release.

Submitting and payment

How to submit it
Return it to the DDS or MDHHS local office that sent it; the form is issued per case and is not posted publicly.
Who pays
Michigan's eligibility manual (BEM 260) says MDHHS helps clients obtain medical evidence when they need it, which can include paying for the medical evidence.

Good to know

  • Michigan's administrative manual (BAM 815) pairs it with the DHS-49-E, Mental Residual Functional Capacity Assessment, for mental health conditions.
  • Michigan policy accepts medical evidence of disability based on the findings of an M.D., D.O. or fully licensed psychologist.
  • After a prior denial, MDHHS requests the DHS-49-D and DHS-49-E to document a new or worsening mental health condition.
  • DDS records its certification and the next medical review date on the DHS-49-A, Medical-Social Eligibility Certification.

Frequently asked questions

Who can complete the Michigan DHS-49-D?

A psychiatrist who is an M.D. or D.O., or a fully licensed psychologist. Michigan's eligibility manual requires medical evidence of disability to be based on the findings of an M.D., D.O. or fully licensed psychologist, and the DHS-49-D is the mental health exam report used for that evidence.

What is the difference between the DHS-49-D and the DHS-49-E?

The DHS-49-D is the examination report: mental status, diagnoses and prognosis. The DHS-49-E is the Mental Residual Functional Capacity Assessment, which addresses the person's remaining mental capacity for work. Michigan policy requests them together for mental health conditions.
