WA DSHS 13-865 Psychological/Psychiatric Evaluation

Washington DSHS uses this evaluation to decide whether a client's mental impairment causes incapacity for Aged, Blind or Disabled cash assistance or a Housing and Essential Needs referral. The examining psychologist, psychiatrist or other qualified professional documents a clinical interview, symptoms, DSM diagnoses and a medical source statement rating 13 basic work activities, then gives a prognosis and estimated duration of impairment.

Who completes it

A licensed psychologist, psychiatrist or other qualified mental health professional paid by DSHS completes the whole evaluation and signs it. The client signs the Section B authorization, or authorizes release on the separate DSHS 14-012 consent.

Submitting and payment

How to submit it
The completed, typed report goes back to the DSHS office named on the form, with all testing documentation and a mental status examination attached.
Who pays
DSHS reimburses the evaluation only if the form is typed or completed in word processing software and complete.

Good to know

  • A handwritten form is not eligible for reimbursement; DSHS posts the 13-865 as a Word file as well as a PDF so it can be typed.
  • A mental status exam following the 13-865 Guidelines must be attached. DSHS states that a mini mental status exam cannot be substituted.
  • Each basic work activity is rated none or mild, moderate, marked, severe or indeterminate, based on sustaining it over a normal workday and workweek, followed by an overall severity rating.
  • The prognosis section asks whether a protective payee is recommended and whether vocational training would reduce barriers to employment.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Does the DSHS 13-865 have to be typed?

Yes. The form states it must be typed or completed using word processing software to be eligible for reimbursement. DSHS publishes a Word version for this purpose. Attach all testing documentation, including subscores, and a mental status exam that follows the 13-865 Guidelines.

What must the mental status exam include?

The 13-865 Guidelines require detailed observations of appearance, speech, attitude and behavior, mood and affect for every client. If not within normal limits, the exam must also detail thought process and content, orientation, perception, memory, fund of knowledge, concentration, abstract thought, and insight and judgment.

Can the client see the 13-865 report?

The form warns that the information may be released to the individual evaluated and is subject to Washington public disclosure laws. The client's authorization covers mental health, substance use, sickle cell disease and sexually transmitted disease information when it is part of the evaluation.
