WA DCYF 13-001B Applicant Mental Health Report

Washington's DCYF Licensing Division uses this confidential report when a foster care or kinship caregiver applicant has a mental health provider. The provider describes past and current diagnoses, current functioning, medications and any concerns about caring for children. It is one part of the licensing review, and DCYF states that no single condition, disability or medication automatically disqualifies an applicant.

Who completes it

The applicant completes Section 1, naming the mental health provider and signing a release, and licensing or child placing agency staff add the licensor's details in Section 2. The mental health provider completes and signs Section 3.

Submitting and payment

How to submit it
The provider returns the report as soon as possible to the licensor's office listed in Section 2. DCYF publishes it as a fillable Word document.
Who pays
The form sets no fee and does not say who pays the provider for the report.

Good to know

  • Section 3 asks for the date first seen, the date of the last appointment and how often the applicant is seen, before the diagnosis questions.
  • For medications, DCYF wants their purpose, problems if not taken as prescribed, and effects on driving safety, alertness and the ability to safely supervise children, including combined effects.
  • The release in Section 1 covers mental health, sexual and physical abuse, substance use and domestic violence history for the lines checked, and stays valid for one year from signature.
  • Complete all of Section 3 even when attaching records. Physical health goes on the companion DCYF 13-001 Applicant Medical Report, completed by the medical provider.
