WA DCYF 13-001 Applicant Medical Report (Foster)

Washington's DCYF Licensing Division sends this confidential report when someone applies to become a foster parent or kinship caregiver. The applicant's medical provider describes chronic issues, current diagnoses, medications and prognosis, and says whether any condition could affect caring for children. Licensing uses it as one part of the overall review; DCYF states that no single condition, disability or medication automatically disqualifies an applicant.

Who completes it

The applicant completes Section 1, naming the provider and signing a release, and licensing or child placing agency staff complete Section 2 with the licensor's contact details. The medical provider completes and signs Section 3.

Submitting and payment

How to submit it
The provider returns the form as soon as possible to the licensor's office listed in Section 2. DCYF publishes it as a Word document, in English and Spanish, so it can be typed before signing.
Who pays
The form sets no fee and does not say who pays for the examination or the report.

Good to know

  • The most recent physical examination recorded in Section 3 must be within 12 months of the application, so an older exam means seeing the applicant again before completing the report.
  • For each current medication, DCYF asks for its purpose, anticipated side effects, concerns if it is not taken, and how it affects daily functioning.
  • The applicant's release covers mental health, sexual and physical abuse, substance use and domestic violence history only for the boxes checked, and it is valid for one year from signature.
  • Complete every Section 3 field even when attaching chart notes, and list any specialist referral with the reason. Mental health providers use the separate DCYF 13-001B.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete DCYF form 13-001?

The applicant's own medical provider completes Section 3: dates of the most recent physical and first visit, chronic issues, diagnoses, medications, prognosis and any concerns about caring for children. The applicant fills in Section 1 and signs the release, and the licensor or child placing agency fills in Section 2.

Is the DCYF Applicant Medical Report available in Spanish?

Yes. DCYF posts the 13-001 as a Word document in English and in Spanish on its forms page. The Word format lets the provider type the answers before printing and signing, and the licensor's contact details in Section 2 show where the finished report goes.

Does a medical condition disqualify a foster care applicant?

DCYF says the report is only one part of the overall review and that no single condition, disability or medication automatically disqualifies an applicant. If Licensing has concerns about safe care, it discusses them with the applicant and tries to resolve them before deciding, so specific, functional comments are most useful.
