SSA-4815: Medical Report on Child with HIV

Social Security field offices send this report to the treating source of a child who alleges HIV infection in an SSI childhood disability claim. The source documents the diagnosis, HIV-related conditions and their effects on growth and function, so SSA can decide the claim and whether to begin presumptive disability payments while the final decision is pending. SSA-4814 is the adult version.

Who completes it

The child's treating pediatrician or other medical source completes the medical sections that follow the parent's release and signs with name and title; a physician, nurse or other acceptable hospital or clinic staff may sign. The parent or guardian signs the release when no SSA-827 is attached.

Submitting and payment

How to submit it
Return it to the SSA field office or State disability determination services shown on the form, by mail in the enclosed return envelope or by fax; the field office may also complete it with you by phone.

Good to know

  • The field office can find presumptive disability when a manifestation block is checked, at least one numbered item is completed, the required section is done, and the source signs with a title.
  • Presumptive payments can run up to 6 months before the final decision and generally are not treated as overpayments if the child is later found not disabled.
  • SSA does not hold the claim waiting for the report; State disability determination services can make a presumptive finding from other evidence and are not limited to the form's categories.
  • The July 2023 edition (OMB 0960-0500) is a nine-page fillable PDF, much longer than the four-page adult SSA-4814.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What is the difference between SSA-4814 and SSA-4815?

Both are Social Security medical reports for claimants alleging HIV infection, under the same OMB number. SSA-4814 is for adults, while SSA-4815 is for children and runs nine pages, adding the growth and functional effects that matter in a childhood SSI claim. Field offices apply the same four completion tests to both for presumptive disability.

How quickly should Form SSA-4815 be returned?

SSA does not set a fixed number of days, but field offices include a return envelope to ensure speedy handling and can take the answers by phone instead. Because a completed report can support presumptive SSI payments for the child, a prompt reply can start benefits before the final decision.
