SSA-787: Patient's Capability to Manage Benefits

SSA sends this three-page form to a beneficiary's physician or other medical source when it must decide whether to pay an adult directly or through a representative payee. The source answers questions about how well they know the patient, the basis for their assessment, and whether the patient can manage or direct the management of funds for basic needs. The determination affects how benefits are paid, not whether the person is disabled.

Who completes it

The physician or other medical source treating the beneficiary completes pages 2 and 3 and prints their name, title and contact details; SSA fills in the identifying block for the patient.

Submitting and payment

How to submit it
Return the completed form to SSA in the enclosed envelope; SSA mails it pre-filled for a specific beneficiary, so there is no public download.
Who pays
SSA states on the form that it cannot compensate the provider for the time spent completing it.

Good to know

  • If you cannot assess the patient's ability to manage funds, answer no to question 4, skip the remaining questions and date the form.
  • SSA notes that needing help with tasks like bill paying does not by itself mean incapability; a person who can direct the management of their benefits is considered capable.
  • SSA weighs the form with lay evidence, such as interview observations and statements from relatives; a medical opinion of incapability does not decide the outcome on its own.
  • Formerly the Physician's/Medical Officer's Statement of Patient's Capability to Manage Benefits, it was retitled in the 2024 revision (OMB 0960-0024).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Does SSA pay for completing Form SSA-787?

No. The form states that SSA cannot compensate the medical source for the time it takes to provide the information. SSA estimates about 20 minutes to read the instructions, gather the facts and answer the questions, and it mails the form with an envelope for returning it to the requesting office.

Does Form SSA-787 affect a patient's disability claim?

No. The form says the capability determination affects only how benefits are paid, directly to the beneficiary or through a representative payee, and has no bearing on disability determinations. Examples of impairments that may cause incapability include dementia, brain damage and chronic schizophrenia.

What counts as being able to manage benefits on SSA-787?

The form asks whether the patient understands their income, assets and expenses, can handle a checking or savings account, and can pay bills on time, then whether they can manage or direct the management of funds to meet basic needs such as food, clothing and shelter. Actual performance, when known, is usually the best indicator.
