SSA-3830: Certification of Low Birth Weight for SSI

SSA sends this two-page form to hospitals to get standardized information for a low birth weight infant's SSI disability claim. Signed by a physician from the medical record, it gives disability determination services enough to decide under listing 100.04 and set the review diary, and lets the field office make a presumptive disability finding. SSA's request letter says a physician's signature allows a faster determination.

Who completes it

A hospital physician, typically from the NICU, completes items 1 to 11 and signs the certification on page 2 from the child's medical record. SSA staff may pre-fill items 1 to 6 before sending it.

Submitting and payment

How to submit it
Return it to the requesting SSA office or disability determination services shown in the For SSA Use Only block; SSA may fax it to speed things up, and a discharge summary may be attached.
Who pays
SSA's cover letter for the form states that it does not pay for completing it.

Good to know

  • Answers to questions 7 and 8 are enough for the field office to establish presumptive disability; question 9 helps set the medical review diary date.
  • Presumptive categories cover birth weight under 1200 grams, or under 2000 grams at 37 to 40 weeks, with lower cutoffs from 32 to 36 weeks.
  • SSA sends the form with a signed SSA-827 authorization and may not alter it; a hospital may answer on its own form or with medical records instead.
  • Presumptive payments can be made for up to 6 months while the formal decision is completed. The current edition is dated July 2023 (OMB 0960-0720).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does a hospital have to use Form SSA-3830?

No. SSA's operations manual says a hospital is not required to use the SSA-3830 if it submits the information on its own form or sends medical records. SSA staff may not alter the form or give it to applicants to take to the hospital; it comes from SSA with a signed SSA-827 authorization.

Which birth weights qualify for presumptive SSI disability?

For a child under one year, field offices can find presumptive disability when evidence shows a birth weight below 1200 grams. For gestational ages of 32 to 40 weeks, the cutoffs rise with age, from 1250 grams or less at 32 weeks to under 2000 grams at 37 to 40 weeks.
