RRB Form SI-7: Supplemental Provider's Statement

Once a railroad employee is receiving sickness benefits, the Railroad Retirement Board mails this form to the treating provider to confirm the disability is continuing. It asks for the last exam date, diagnosis, current objective findings, complications, response to treatment, surgery, maternity dates, an estimated return-to-work date and whether maximum medical recovery has been reached.

Who completes it

The health care provider treating the employee completes items 1 through 7 and signs with degree or title and National Provider Identifier. The patient's name and Social Security number appear at the top.

Submitting and payment

How to submit it
Mail it to the RRB immediately in the enclosed envelope; the return address is the RRB's RUIA unit in Houston.
Who pays
The RRB states that the information is to be supplied without expense to it.

Good to know

  • No additional sickness benefits can be paid to the patient until the supplemental form is completed and returned.
  • If the patient still can't work, give an estimated return-to-work date and explain how the medical evidence shows continuing disability; an indefinite date still needs an estimate.
  • If the patient has reached maximum medical recovery, give the date and say whether the patient can do some kind of work.
  • It follows the initial SI-1b Statement of Sickness; the current edition is SI-7 (08-23) under OMB 3220-0039.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Why did I receive RRB Form SI-7?

The Railroad Retirement Board needs additional medical evidence before it pays further sickness benefits under the Railroad Unemployment Insurance Act. It usually mails the SI-7 to the treating provider when benefits continue, with an envelope for returning it. No more benefits are paid until it comes back.

What does Form SI-7 ask about return to work?

Item 5 asks whether the patient can now work without restriction in their last occupation. If yes, give the date they became able to work. If no, give an estimated return-to-work date and explain how the medical evidence shows the patient is still disabled.
