RRB Form SI-1b: Statement of Sickness

A railroad employee applying for sickness benefits under the Railroad Unemployment Insurance Act needs medical evidence on this one-page statement, the last page of the SI-1a application packet. It records when the patient became sick, examination dates, diagnosis and concurrent conditions, any surgery or hospital stay, maternity dates and the date the patient should be able to resume work.

Who completes it

A provider trained in medical, surgical, dental or psychological diagnosis, a certified nurse-midwife for pregnancy or childbirth, a hospital supervisory official, a chiropractor, a certified PA or a nurse practitioner completes all of SI-1b. The employee completes the SI-1a application.

Submitting and payment

How to submit it
Complete it and return it to the patient immediately so they can mail it with the SI-1a to the RRB's RUIA unit in Houston; delays can cost the patient benefits. Extra medical records may go straight to the RRB, marked with the patient's name and Social Security number.
Who pays
The form states the RRB is not liable for any charge for completing it.

Good to know

  • If the return-to-work date is indefinite or unknown, item 8 still asks you to give an estimated date.
  • Sign with your degree or title and National Provider Identifier; the certification carries criminal and civil penalties for false or fraudulent statements.
  • If the patient can't sign forms, also complete Section 2 of the SI-10 in the same packet.
  • Employees can apply online through myRRB, but the provider statement stays on paper; when benefits continue, the RRB later sends the SI-7 supplemental statement.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
