RRB Form G-260: Report of Seizure Disorder

When a railroad worker's disability annuity claim rests on epilepsy or another seizure disorder, the Railroad Retirement Board asks the treating physician for this report. It records a description and type of the seizures, first and last seizure dates, frequency over the past year and who has verified it, precipitating factors, treatment, compliance, blood drug levels, mental status, postictal behavior, neurological findings and EEG results.

Who completes it

The treating physician or neurologist, called the medical examiner on the form, completes the whole report and signs the Section 15 certification. The applicant does not complete any part.

Submitting and payment

How to submit it
Return it promptly, with your narrative report, copies of office records and the claimant's RRB claim number, to the RRB address on the form. A narrative on your own stationery may replace the form if it covers every item.
Who pays
Because applicants present medical evidence from their own physicians, the RRB says any fee is a personal matter between applicant and physician unless it contracts for the exam.

Good to know

  • Attach a copy of the EEG, or say where the RRB can obtain it, and state whether the report is compiled from records or based on a new examination.
  • Item 10 asks how the seizure dates in the past year are known and whether anyone other than the applicant has verified them.
  • Print in ink, enter dates as numbers with one digit per box, and use item 21 for any further history or description.
  • The current edition is Form G-260 (01-21), which says to destroy prior editions; it is collected under OMB 3220-0038.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can I write a letter instead of filling in RRB Form G-260?

Yes. The form says your report may be made on the form or as a narrative on your own stationery. The narrative must still furnish all the information the form requests about the applicant's seizure disorder, including treatment, compliance, blood drug levels and EEG findings.

Who pays for completing RRB Form G-260?

The RRB explains that applicants are responsible for presenting medical evidence from their own physicians, so any fee for completing the report is a personal matter between the applicant and the physician. The exception is when the RRB specifically contracts for an examination.
