RRB Form G-250: Medical Assessment for Disability

The Railroad Retirement Board sends this body-system assessment to a railroad employee's treating physician when the employee applies for a disability annuity under the Railroad Retirement Act. It walks through musculoskeletal findings with range-of-motion tables, then cardiovascular, respiratory, neurological, vision, hearing, speech and mental findings, and ends with exertional and environmental restrictions that the RRB uses to decide the claim.

Who completes it

The patient's treating physician completes the whole form, from patient identification through the Section 13 certification, and signs it. The employee does not fill in any part.

Submitting and payment

How to submit it
Mail the signed form with copies of your office records to the RRB Office of Programs in Chicago, as printed on the form. It is a paper form that the RRB mails out per case with cover letter RL-250.
Who pays
The G-250 says nothing about payment; on its companion G-260 the RRB states report fees are a personal matter between applicant and physician unless it contracts for the exam.

Good to know

  • The report must be signed by hand: the certification states that a stamped signature is not acceptable.
  • Follow the Go to instructions, and enter NA for not affected or UNK for unknown rather than skipping items you were not directed to skip.
  • Attach the results the form asks for: x-ray, MRI and CT reports, EKG tracings, pulmonary function tests, visual field charts and audiometric charts.
  • The current edition is G-250 (01-21), which says to destroy prior editions; the RRB often sends it with G-250A for residual functional capacity.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Where does RRB Form G-250 come from?

The Railroad Retirement Board mails it to the treating physician with cover letter RL-250 when a railroad employee claims a disability annuity. It is not posted on rrb.gov, so physicians normally receive it for a specific patient rather than downloading a blank copy.

Can I sign Form G-250 with a signature stamp?

No. The Section 13 certification says the report must be signed and that a stamped signature is not acceptable. Print your name and title, add your National Provider Identifier, and give an address and daytime telephone number so the RRB can follow up.

What should I attach to Form G-250?

Return it with copies of your office records. Where a system is abnormal, the form also asks for imaging reports for musculoskeletal findings, EKG tracings for cardiac findings, pulmonary function tracings, visual field charts and audiometric charts.
