DC Universal Health Certificate (UHC)

D.C. Code 38-602 requires a Universal Health Certificate for every child attending DC public, charter and private schools, child care and youth programs, renewed every year. It records the physical exam, vision and hearing screening, health concerns, TB risk, lead testing, immunizations and exemptions. The completed certificate goes to the school or child care facility.

Who completes it

The parent completes Part 1 and signs the release. A licensed health care provider completes Parts 2 to 5: exam and screenings, immunizations, sexual history for grades 9 to 12, and certifications, with signature and office stamp.

Submitting and payment

How to submit it
Return it to the parent, who gives it to the school health suite or child care facility every year; school staff record its receipt in an office-use box.

Good to know

  • Children 3 to 6 need only pass or fail for vision; those 6 and over need acuity levels. Every child must have 2 lead tests by age 2.
  • High TB risk calls for a skin test and/or IGRA, and positive TB tests should be referred to the primary care provider for evaluation.
  • Medical exemptions, permanent or temporary, and laboratory proof of immunity with titer results attached are recorded on page 4.
  • If the child takes medication or needs treatment at school, attach a completed Medication/Medical Treatment Plan form.
