Montana Health Care/Medication Plan (Child Care)

Montana licensed child care providers use this two-page plan for a child with special health needs or medication given during care, and the child's health care provider must approve it. It tells caregivers which medications to give, what accommodations the child needs through the day, when to call parents or 911, and what training staff may need.

Who completes it

The plan must be approved and signed by the child's health care provider. The parent signs consent for the provider or specialist to talk with the child care provider and may add notes.

Submitting and payment

How to submit it
The signed plan is returned to the child care provider, who keeps it so every caregiver knows the child's needs.
Who pays
The form sets no fee and does not address payment for completing the plan.

Good to know

  • Emergency care has three parts: symptoms that mean calling parents, symptoms that mean calling 911 as well as parents, and measures to take while waiting for help.
  • The plan also lists special equipment or medical supplies and suggested special training for staff; the current version is dated June 2023.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who must approve a Montana child care health care plan?

The form is headed 'To be approved by a Health Care Provider' and has a health care provider signature and date line on page two. The parent signs separately to allow the provider or specialist to communicate with the child care provider.

What contacts does the Montana medication plan ask for?

The top of the plan lists the parent or guardian, the primary health care provider and specialty providers with their specialties and telephone numbers, followed by the child's diagnoses and allergies.

Is the Montana Health Care/Medication Plan the same as a medication authorization?

No. Montana child care licensing lists a separate Medication Authorization Form. This plan covers the broader care of a child with special health needs, including routine medications, accommodations, equipment, emergency steps and staff training.
