DoDEA Form 1307: Sure Start Medical/Dental Exam

DoDEA's Sure Start program requires each enrolling child to have a medical and dental examination certifying satisfactory health and freedom from communicable disease, and Form 1307 records both. A 4-year-old well-child check can be used for the medical section, or completed if there is no record of one. The form goes to the DoDEA school where the child is enrolling.

Who completes it

The parent or sponsor completes Section I and signs the Section IV authorization. The physician or primary care manager completes Section II, the medical exam, and a dentist completes Section III, the dental exam, each with signature and stamp.

Frequently asked questions

Can a 4-year well-child visit be used for DoDEA Form 1307?

Yes. Section II says that if a 4-year-old well-child check-up has been completed, its results may be used to complete the form. If there is no record of one, the physician or primary care manager should complete the 4-year-old well-child check-up.

Who completes the dental part of DoDEA Form 1307?

A dentist completes Section III after a complete dental examination, marking that the child is in satisfactory health and free from communicable disease, listing any limitations, and signing with a stamp. The medical exam in Section II is completed separately by the physician or primary care manager.
