Wisconsin DCF-F-CFS0060 Child Health Report

Children enrolling in a Wisconsin licensed child care center need a health exam on file, and this DCF report is the standard way to provide it. The provider records care instructions for special health concerns, milk and other allergies with a treatment plan, the most recent blood lead test date and any medically contraindicated immunizations, then certifies the child can take part in child care activities.

Who completes it

A physician, physician assistant or other EPSDT (HealthCheck) provider completes the Health Professional section and signs the authorization. The parent or guardian fills in the child and parent details at the top.

Submitting and payment

How to submit it
The parent or guardian takes the form to the provider and then gives the signed copy to the center, whose licensee keeps a copy in the child's record.
Who pays
The form sets no fee and does not say who pays for the examination.

Good to know

  • Children under 2 need an exam no more than 6 months before or 3 months after admission, then a follow-up at least every 6 months.
  • Children aged 2 until age 5 need an exam no more than one year before or 3 months after admission, then a follow-up at least every 2 years.
  • The form is required unless the exam report is an electronic printout from a licensed physician, physician assistant or other EPSDT provider.
  • Immunization documentation must also be on file at the center; the form suggests parents include a copy of the record, and DHS F-44192 is the companion record.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the Wisconsin Child Health Report available in Spanish?

Yes. DCF posts a Spanish edition, Informe de Salud Infantil - Centros de Cuidado Infantil, next to the English DCF-F-CFS0060-E on its child care licensing forms page. Families can bring either version to the child's physician, physician assistant or HealthCheck provider.

How often does a child in Wisconsin child care need a new health report?

It depends on age. Under age 2, a follow-up exam is due at least every 6 months after the initial exam. From age 2 until age 5, a follow-up is due at least every 2 years. The initial exam can fall up to 6 months (under 2) or one year (age 2 to 5) before admission.

Does the Child Health Report require a lead test?

It asks for the date of the child's most recent blood lead test. The form notes that children on Medicaid must be tested at about 12 and 24 months, or once between ages 3 and 5 if no earlier test is documented, and that lead testing is optional for children not on Medicaid.
