Michigan CCL-3704 Medical Clearance Request

Michigan child care licensing uses this request when it needs a physician's view on whether an applicant, licensee or other person has a physical or mental condition that could affect the care of children. The physician describes the person's general physical and mental health and chooses one of two statements about their ability to provide independent care for children from birth to age 17.

Who completes it

The applicant or licensee fills in the facility or home name and license number, and the patient completes their own details and signs the release of information. The physician completes the medical information section and signs it.

Submitting and payment

How to submit it
The physician returns the completed form to the applicant or licensee, who provides it to the MiLEAP child care licensing consultant.
Who pays
The form sets no fee and does not address who pays for the physician's evaluation.

Good to know

  • One statement says no condition limits the ability to provide independent care; the other says a condition would affect that ability, with or without reasonable accommodation, explained in the comments.
  • The release authorizes medical information to go to the child care home and to MiLEAP solely to determine suitability to provide or be associated with the care of children.
  • The physician can check whether they would like the licensing consultant to contact them about the recommendation.
  • The form cites 1973 PA 116; completion is voluntary, but an application for licensure may be denied without it. The current version is CCL-3704 (Rev. 04/2026).

Frequently asked questions

When does Michigan child care licensing ask for a medical clearance?

Licensing sends CCL-3704 when it needs a physician's statement about whether a person caring for children, or associated with a child care home, has a physical or mental condition or health problem that could affect the health or safety of children or the quality of their care.

What does the physician decide on CCL-3704?

The physician describes the person's general physical and mental condition, then checks either that no condition limits their ability to provide independent care of children from birth to 17, or that a condition would affect that ability, noting any reasonable accommodation in the comments.

Which agency uses the Michigan Medical Clearance Request?

The form is issued by Child Care Licensing in the Michigan Department of Lifelong Education, Advancement, and Potential (MiLEAP). It was formerly numbered BCAL-3704 and DHS-OCAL-3704, and previous editions are obsolete.
