Kansas CCL 029a Child Health Assessment (Child Care)

Kansas child care licensing requires a health assessment for every enrolled child, including the provider's and staff members' own children, in family child care homes, centers and preschools. This KDHE form records the history, growth measurements, physical exam and screening results the facility keeps on file so caregivers know about allergies, medications and special care needs.

Who completes it

A licensed physician, a physician assistant or a nurse approved to perform health assessments completes and signs the form. It has no parent section; the child care facility keeps the signed form in the child's record.

Submitting and payment

How to submit it
The signed form goes back to the family for the child care facility's file. The assessment must be done no more than 12 months before and no later than 60 calendar days after enrollment.
Who pays
KDHE sets no fee on the form and does not address who pays for the assessment.

Good to know

  • The physical exam grid covers HEENT, teeth, cardio/respiratory, abdomen/GI, genitalia/breasts, extremities/joints/back/chest, skin/lymph nodes, and neurologic and developmental findings, with a comment line for anything abnormal.
  • Screening rows for lead, anemia (HGB/HCT), urinalysis, hearing and vision ask for the screening date and a note if results are pending or abnormal.
  • Length or height and weight are recorded with percentiles, and the provider indicates whether they see the child for regular health supervision.
  • The current printing is marked Rev. 02/2026; a companion form, CCL 029, records the child's immunization history.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
