American Lung Association My Asthma Action Plan

For adults and families managing asthma day to day, the American Lung Association's plan lays out green, yellow and red zones, with controller and quick-relief medicines, doses and timing in each zone, including SMART/MART dosing with an ICS-formoterol inhaler. It lists triggers, severity, vaccination status, danger signs that call for 911, and emergency contacts. The second page shows how to use a metered-dose inhaler with a spacer.

Who completes it

The patient's health care provider fills in the plan with the patient, including severity, triggers, medicines and zones. This general version has no signature block; the home and school version carries the signatures.

Submitting and payment

How to submit it
Give the completed plan to the patient to keep and share with caregivers or employers as needed; for school, ALA offers a separate home and school version with signatures and self-carry language.

Good to know

  • SMART/MART lines cap daily ICS-formoterol use at 12 puffs for ages 12 and up and 8 puffs for ages 4 to 11, taken with a spacer.
  • Call 911 if the patient has trouble walking or talking from shortness of breath, blue lips or fingernails, or is still in the red zone after 15 minutes.
  • The plan says patients should feel better within 20 to 60 minutes of quick-relief treatment; if worse, they follow the red zone and call the doctor or 911.
  • The July 2025 edition is a flat PDF in English and Spanish (Mi Plan de accion de asma), with Hebrew, Haitian Creole, Russian and Ukrainian versions.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
