Emergency Information Form for CSHCN (AAP/ACEP)

Pediatric and emergency medicine groups created this form so a child with special health care needs gets prompt, appropriate care on arriving at an emergency department or other setting with an acute illness or injury, when neither a parent nor the child's pediatrician is available. It concisely summarizes diagnoses, baseline findings, medications, allergies and emergency management for physicians, parents, EMS professionals and nurses.

Who completes it

The child's primary care physician or specialist completes the whole form with the parent and signs it.

Submitting and payment

How to submit it
Give the completed form to the family to keep, with copies for the school, child care program, local EMS and hospital.

Good to know

  • ACEP offers the blank form as a PDF and as an interactive Word document that can be filled in electronically, plus a sample completed form to show the expected detail.
  • The EIF is presented jointly by ACEP and the AAP, backed by ACEP's policy on access to critical health information and the AAP statement on emergency preparedness for these children.
  • The posted PDF is a scanned image, so it is completed by hand or through the interactive Word version rather than as a fillable PDF.
  • Several state Emergency Medical Services for Children programs also host copies of the form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
