AAAAI Asthma Action Plan for Home & School

AAAAI designed this two-page plan for children whose asthma is managed both at home and in school. It sets green, yellow and red zone instructions, including controller medicines given at school, rescue and exercise medicine, and when to call the doctor or 911, and its second page makes it a school document: provider and parent decide on self-carry, the parent authorizes medication at school, and the school nurse records a review.

Who completes it

The child's asthma provider completes page 1 (severity, zones, medicines and triggers) and signs page 2 with printed name and contact information. The parent or guardian signs the medication authorization and communication consent, and the school nurse records their review.

Submitting and payment

How to submit it
Give the signed plan to the parent for the school; the form asks that a signed copy be sent back to the provider listed on it.

Good to know

  • Unless noted otherwise, the only controllers given at school are those listed as given in school in the green zone; staff follow the yellow and red zones for rescue medicine.
  • Self-carry needs both the asthma provider and the parent to agree that the child may carry and self-administer inhalers, with a separate box for the school nurse's agreement.
  • If the child is in the yellow zone more than 24 hours or getting worse, the plan says to follow the red zone and call the doctor right away.
  • The parent's consent covers communication between the prescriber, school nurse, school medical advisor and school-based health clinic. The edition is dated September 29, 2022 (AAAAI-0922-516).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a student carry their own inhaler under the AAAAI plan?

The plan has a checkbox for the asthma provider and parent to agree that the child may carry and self-administer their inhalers, and a separate checkbox for the school nurse to agree. Both appear on page 2, next to the provider signature and the parent's medication authorization.

Which asthma medicines can school staff give under the AAAAI plan?

Staff follow the yellow and red zone plans for rescue medicines according to symptoms. Unless otherwise noted, the only controller medicines given at school are those the provider lists as given in school in the green zone, and the parent's signature authorizes the nurse or other school staff to give them.
