New Jersey TB-70 Tuberculosis Case and Status Report

New Jersey reports TB suspects and cases, and their initial, current and final status, on this state version of CDC's Report of Verified Case of Tuberculosis (RVCT). The five-page fillable form (FEB 22) follows the patient from diagnosis through treatment: risk factors, test results, imaging, site of disease, drug regimen, susceptibility testing and outcome.

Who completes it

The treating physician or chest clinic and local health department TB program staff complete it, with the preparer named in item 33; the GEOID and state case number are assigned at the state level.

Submitting and payment

How to submit it
Send it to the NJDOH Tuberculosis Program through the local health department TB program. To get a state case number, contact the TB Program with the patient's name, date of birth and county.
Who pays
NJDOH lists no fee or billing code for the report.

Good to know

  • Count status options are suspect (pending verification or labs), recurrent within 12 months of completing therapy, diagnosed as TB by provider, MTB positive lab, or not TB.
  • Always enter the initial TST, IGRA, sputum smear, sputum culture, NAAT and HIV test, writing 'Not Done' for any test that was not performed.
  • A TB-41 contact investigation form must be submitted for all infectious cases, and the MDR TB Supplemental Surveillance Form for any case treated as MDR.
  • Leave pending items blank; if a value can't be determined and there is no Unknown box, write Unknown inside the item.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the TB-70 the same as the CDC RVCT?

It collects the same data. The RVCT is CDC's national TB surveillance form, and some states use a modified version or their own tool; in New Jersey it is called the TB-70. The FEB 22 version reflects the expanded RVCT whose new variables began to be collected in January 2022.

When is a recurrent TB patient counted as a new case?

Per the TB-70 instructions, a patient who recurs within 12 months of completing therapy keeps the previous state case number, while one who recurs after more than 12 months is treated as a separate episode and counted as a new case. A case is not counted twice within any consecutive 12-month period.
