New Jersey CDC-2 Rabies Post-Exposure Treatment

When a New Jersey patient starts rabies post-exposure treatment, the treating provider reports it on this form. It records the exposure type, body part, animal, circumstances, the animal's rabies status and the treatment given, and goes to the Health Officer of the patient's municipality, who forwards a copy to NJDOH's Communicable Disease Service. Here 'CDC' is New Jersey's own prefix, not the federal agency.

Who completes it

The treating health care provider completes the patient, exposure and treatment fields and signs. The local Health Officer adds their name, the health department and the date initially reported.

Submitting and payment

How to submit it
Fax or mail the form to the Health Officer where the patient resides, or relay the same information by phone. The Health Officer keeps a copy and forwards the form to NJDOH by fax or mail.
Who pays
NJDOH lists no fee or billing code for the report.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
