New Jersey STD-11a Syphilis Case Report Form

New Jersey providers who cannot enter a syphilis case directly in CDRSS, the state's preferred route, report it on this form. It records the patient, pregnancy and estimated delivery date, the stage (primary through late latent, unknown duration or congenital), neuro, ocular or otic involvement, symptoms, and prior syphilis testing and treatment, and must be faxed with the lab report.

Who completes it

The examining provider or clinic staff completes it, and the reporting clinic block names both the person completing the form and the examining provider. Questions printed in red are required.

Submitting and payment

How to submit it
Fax it with the lab report to the NJDOH STD Program's confidential fax line when CDRSS entry isn't possible. The STD Program can also take reports by phone.
Who pays
NJDOH lists no fee or billing code for the report.

Good to know

  • The lab report must be faxed with the form; a case report sent without it is incomplete.
  • Chancroid is not reported on the STD-11 forms; call the STD Program to discuss a case.
  • The form asks about the last HIV test and result, whether the patient is on PrEP and whether they were referred to PrEP.
  • Gonorrhea is reported on STD-11b and chlamydia on STD-11c, both faxed to the same STD Program line.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Do I need to fax the lab report with the New Jersey syphilis form?

Yes. The STD-11a states the lab report must be faxed with the case report form, and the lab results section repeats it. Required questions are printed in red, and NJDOH asks that all required fields be complete before you submit the form.

Who must report syphilis in New Jersey?

Physicians, advanced practice nurses and physician assistants, plus those who oversee hospitals, health care facilities, correctional facilities, schools, camps, child care centers, preschools and colleges. Laboratories follow their own rules under N.J.A.C. 8:57-1.7. Direct entry in CDRSS is preferred over the paper form.
