New Jersey CDS-1 Communicable Disease Report

New Jersey providers use this paper report for a reportable communicable disease under N.J.A.C. 8:57, sent to the local health department where the patient resides. NJDOH now prefers electronic case reporting or direct entry in CDRSS, so the one-page CDS-1 is the fallback when a provider is not set up for those. It covers the patient, setting, case status, labs and clinical information.

Who completes it

The provider (physician, APN, PA or facility staff) completes the form except the shaded areas, which are for the local health department, including the CDRS ID and the reporting health officer.

Submitting and payment

How to submit it
Fax or securely email the form to the local health department where the patient resides. Immediately reportable conditions are phoned to the local health department, with an NJDOH backup line, and others are reported by the next business day.
Who pays
NJDOH lists no fee or billing code for the report.

Good to know

  • Setting of infection is marked as a sporadic case, household cluster, institutional cluster or outbreak, and case status as possible, probable or confirmed.
  • The form asks about travel, blood transfusion or renal dialysis in the past six months, and vaccination status for vaccine-preventable diseases.
  • Lab sections cover culture, antigen tests and serology, with first and second blood dates and titers for paired serology.
  • Many diseases have their own New Jersey case report forms, such as CDS-17 for hepatitis C, CDS-40 for STEC and IMM-23 for meningococcal disease.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Is the CDS-1 still used in New Jersey?

Only as a fallback. NJDOH prefers electronic case reporting (eCR) or direct entry in its Communicable Disease Reporting and Surveillance System (CDRSS). Providers not authorized for those fax or securely email the CDS-1 to the local health department where the patient resides.

Which local health department gets the CDS-1?

The one serving the municipality where the patient resides. The form is headed 'For Submission to Local Health Department', and the health department completes the shaded areas, assigns a CDRS ID and records the reporting health officer representative and the date initially reported.
