Louisiana DCFS Mandated Reporter Portal (Online Report)

Louisiana DCFS's Mandated Reporter Portal lets mandated reporters file non-emergency reports of suspected child abuse or neglect online, 24 hours a day, and submit the written follow-up required within five days of an oral report. A clinician can use it when the child does not need immediate help. DCFS's 22-page online reporting guide walks through each screen with examples of sufficient and insufficient reports.

Who completes it

The mandated reporter completes the whole online report: reporter information, location, each child, the adults involved and the abuse or neglect details. Medical personnel are asked to include diagnoses and necessary treatment.

Submitting and payment

How to submit it
Submit the report through the DCFS Mandated Reporter Portal, linked from the DCFS mandated reporters page. Child fatalities, life-threatening injuries, drug-exposed newborns, human trafficking, Safe Haven relinquishments, sexual abuse and children in immediate danger must be called in to the DCFS Child Abuse Hotline instead.
Who pays
DCFS lists no fee or payment for reports.

Good to know

  • DCFS warns that entering an emergency report in the portal may delay its response, so the portal opens with acknowledgement questions that screen out hotline-only cases.
  • Medical reporters should state whether the child needs medical attention and whether there is a health risk if recommended medication or treatment is not received.
  • DCFS investigates only parents and caretakers as defined in the Children's Code, so describe the alleged perpetrator's relationship to the child.
  • Add an email address in the reporter section if you want confirmation that the report was submitted successfully.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Which reports cannot go through Louisiana's Mandated Reporter Portal?

Reports about a child fatality, a life-threatening injury, a drug-exposed newborn, human trafficking, a Safe Haven relinquishment, sexual abuse, or a child in immediate danger. DCFS's guide says to call the Child Abuse Hotline for these, because entering an emergency in the portal may delay the response.

Does a portal report count as the five-day written report?

Yes. DCFS states that oral reports must be followed by a written report within five days, and that this can be done by submitting a follow-up online in the Mandated Reporter Portal or by mailing the CPI-2 form to the Louisiana Child Safety Response Center.

What details does DCFS want in a portal report?

The child's name, address, age, sex and race, any special needs, the nature, extent and cause of injuries, the caretakers and household members, the child's current location, how the situation came to your attention, any explanation given, the suspected perpetrator, and current safety concerns. DCFS asks reporters to be as specific as possible.
