Louisiana CPI-2 Mandated Reporter Written Report Form

CPI-2 is the Louisiana Department of Children and Family Services form for a mandated reporter's written report of suspected child abuse or neglect. Children's Code Article 610 D requires a written report to DCFS or law enforcement within five days of an initial oral report, and CPI-2 can also serve as the first report. It covers the victims, caretakers, suspected perpetrator, injuries and family strengths.

Who completes it

The mandated reporter completes and signs the whole form, entering Unknown for items they have no information on, and states their position and agency.

Submitting and payment

How to submit it
Mail CPI-2 to the Louisiana Child Safety Response Center (the form also allows faxing or mailing to the DCFS parish office), or file the follow-up online through the DCFS Mandated Reporter Portal instead. Immediate concerns go to the 24/7 Child Abuse Hotline by phone.
Who pays
DCFS lists no fee or payment for the report.

Good to know

  • Mark whether this is an initial written report or a follow-up to an oral report, with the date and the DCFS office or law enforcement agency you reported to.
  • You do not need to gather every item: complete what you know, write Unknown elsewhere, and add a page if you need more space.
  • Page 2 asks what is going well for the family and whether there was a time they adequately cared for or protected the child.
  • Note any known danger to a DCFS worker, and any previous reports you made about this child or siblings, with dates and outcomes.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long do I have to send the CPI-2 after calling DCFS?

Five days. Louisiana Children's Code Article 610 D requires a mandated reporter who made an oral report to send a written report to DCFS or law enforcement within five days. CPI-2 is one way to meet that requirement; a follow-up report entered in the DCFS Mandated Reporter Portal is the other.

Can I report in writing without calling DCFS first?

Yes. The CPI-2 text states that a mandated reporter may report suspected abuse or neglect in writing instead of making an oral report, and asks reporters who have not yet reported to send it as soon as possible. DCFS directs immediate concerns to its 24/7 Child Abuse Hotline.
