Forest Service Vision Form: Fire Medical Waiver

Wildland firefighters whose OF-178 exam or screening shows a vision deficit take this four-page template to their eye care provider. It records diagnosis, stability, medications, recent distant and near acuity, temporal peripheral vision and eye surgeries, plus concerns about smoke, low light and driving, so a Forest Service Medical Officer can decide clearance or a waiver.

Who completes it

The employee completes Part A with fitness level and identifying details, which authorizes release to the program's Medical Officers. The ophthalmologist, optometrist or treating provider completes Part B as applicable and signs with credentials and an office stamp.

Submitting and payment

How to submit it
The provider faxes the completed form to the Fire Medical Qualifications Program office at the number printed on Part B.
Who pays
No fee schedule applies; the employee is responsible for obtaining and paying for vision documentation the Medical Officer requests.

Good to know

  • Acuity must be measured since the last OF-178 exam or within the last six months: distant and near, corrected and uncorrected, for each eye and both eyes.
  • Temporal peripheral vision must be measured since the last OF-178; Arduous positions require 85 degrees or more and Light or Moderate positions 70 degrees or more.
  • List eye surgeries with the surgery date and the date the employee returned to full, unrestricted work duties.
  • The concern question covers bright and very low light, smoky poor visibility, ledges and heights, narrow dirt roads with drop-offs and using chainsaws or log chippers.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What peripheral vision does the Forest Service require for firefighters?

The vision template states that Arduous positions require temporal peripheral vision of 85 degrees or greater, and Light or Moderate positions require 70 degrees or greater. The provider records each eye's measurement, taken since the employee's last OF-178 exam.

Can old eye exam results be used on the vision template?

Visual acuity must have been performed since the last OF-178 exam or within the last six months, and peripheral vision since the last OF-178. Older results do not meet the form's instructions, so a current measurement may be needed before completing Part B.
