OSHA HAZWOPER Physician's Written Opinion

Hazardous waste workers, emergency responders with possible overexposure and HAZMAT team members need medical surveillance under OSHA's HAZWOPER standard, and each exam ends with a physician's written opinion for the employer. It addresses risk from hazardous waste work, emergency response or respirator use and any recommended limitations. OSHA prescribes the content in 29 CFR 1910.120(f)(7) but no form.

Who completes it

The attending physician, a licensed physician preferably with occupational medicine knowledge, writes the entire opinion after the surveillance exam.

Submitting and payment

How to submit it
The employer obtains the written opinion from the physician and must furnish a copy to the employee.
Who pays
The employer pays; HAZWOPER exams must be provided at no cost to the employee, without loss of pay and at a reasonable time and place.

Good to know

  • The opinion states whether conditions increase the employee's risk from hazardous waste work, emergency response or respirator use, plus any recommended limits on assigned work.
  • Exam and test results are included only if the employee requests them; the opinion must not reveal findings unrelated to occupational exposures.
  • Exams are due before assignment, at least every 12 months (a physician may extend to two years), at termination and as soon as possible after symptoms or overexposure.
  • Covered workers include those exposed at or above limits, or wearing respirators, 30 or more days a year; construction work follows 29 CFR 1926.65.

There is no form to fill: the program asks for a letter or report with the content it sets. Vero Scribe writes the visit note it draws on, so the details are ready when you write it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How often are HAZWOPER medical exams required?

Before assignment, then at least once every 12 months, unless the attending physician believes a longer interval, up to two years, is appropriate. Exams are also required at termination or reassignment out of coverage, unless one was done within six months, and as soon as possible after reported symptoms, overexposure or injury.

What information does the employer give the HAZWOPER physician?

A copy of the standard and its appendices, plus for each employee a description of duties related to exposure, actual or anticipated exposure levels, protective equipment used or to be used, information from prior exams not readily available to the physician, and information required by the respiratory protection standard.
