OSHA Bloodborne Pathogens Written Opinion (HBV)

After a needlestick or other exposure incident, or when hepatitis B vaccination is evaluated, OSHA's bloodborne pathogens standard requires the evaluating healthcare professional to give the employer a short written opinion. OSHA prescribes no form; the content is set by 29 CFR 1910.1030(f)(5) and deliberately limited so the employee's other diagnoses stay confidential.

Who completes it

The healthcare professional who performed the vaccination evaluation or the post-exposure evaluation writes the entire opinion; the employee does not complete any part of it.

Submitting and payment

How to submit it
The employer obtains the opinion from the healthcare professional and must give the employee a copy within 15 days of completion of the evaluation.
Who pays
The employer pays for hepatitis B vaccination and post-exposure evaluation and follow-up, including the written opinion.

Good to know

  • For vaccination, the opinion is limited to whether hepatitis B vaccination is indicated for the employee and whether the employee has received it.
  • For post-exposure evaluation, it states only that the employee was informed of the results and told about conditions from the exposure needing further evaluation or treatment.
  • All other findings or diagnoses must remain confidential and may not be included in the written report.
  • The employer must give the professional the regulation, the employee's duties, the exposure route and circumstances, source test results if available, and vaccination records.

Get the note ready with Vero

There is no form to fill: the program asks for a letter or report with the content it sets. Vero Scribe writes the visit note it draws on, so the details are ready when you write it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there an OSHA form for the bloodborne pathogens written opinion?

No. OSHA sets the required content in 29 CFR 1910.1030(f)(5) but does not publish a form, so occupational health clinics use their own templates. The opinion must be limited to the permitted items: vaccine indication and receipt, or that the employee was informed of post-exposure results and follow-up needs.

Can the written opinion mention the employee's HIV or hepatitis test results?

No. Beyond the limited statements the standard allows, all other findings or diagnoses must remain confidential and stay out of the written report. The employee learns their results from the evaluating professional; the employer's copy only confirms the employee was informed and told about needed follow-up.
