Forest Service Psychiatric Form: Fire Medical Waiver

When a wildland firefighter's screening or OF-178 exam notes a psychiatric diagnosis or medication, the Forest Service asks the treating provider to complete this template. It documents diagnosis, stability, medication history, recent dose changes, supply planning for assignments and any safety concerns, and a Forest Service Medical Officer uses it to decide medical clearance or a waiver.

Who completes it

The employee completes Part A, including fitness level, and authorizes release to the program's Medical Officers. The treating provider (MD, DO, NP, PA or other) completes Part B as applicable, then signs with credentials and an office stamp.

Submitting and payment

How to submit it
The provider faxes the completed template to the Fire Medical Qualifications Program office at the number printed on Part B.
Who pays
No fee schedule applies; the Forest Service tells employees that obtaining, paying for and supplying requested medical documentation is their responsibility.

Good to know

  • The medication list asks for each drug's indication and the estimated year it was started for the diagnosis.
  • Report any dosage change within the past six months, naming the medication and when the change occurred.
  • The form asks whether the employee has a plan to carry enough medication for a whole assignment plus travel, a minimum of 30 days.
  • For substance use disorder, the provider states whether remission or sobriety has been reached; another item covers effects on driving, heavy equipment or chainsaws.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What safety question does the psychiatric template ask the provider?

It asks whether the provider has concerns about the employee's ability to function in a wildland fire environment and respond appropriately to protect their own life or another person's. A separate item asks whether the condition or medications are likely to affect safe operation of a motor vehicle, heavy equipment or powered equipment such as chainsaws.

Why does the form ask about a 30-day medication supply?

Fire assignments can last up to 21 days in a row plus travel, often far from pharmacies. The template asks whether the employee has a plan to keep a sufficient supply of medication for the whole assignment, a minimum of 30 days, and, if not, asks the provider to document the plan.
