Forest Service Prosthetic Form: Fire Medical Waiver

Employees who use a prosthesis and want a Forest Service fire qualification bring this template to their treating provider. It asks about the prosthesis type and location, power needs, how it holds up to embers and heat, its fit with agency fire clothing, and the employee's mobility on rough ground, so a Medical Officer can decide clearance, a waiver or accommodations.

Who completes it

The employee completes Part A (name, date of birth, home unit and Light, Moderate or Arduous level). The treating provider (MD, DO, NP, PA or other) completes Part B as applicable and signs with credentials and an office stamp.

Submitting and payment

How to submit it
Fax the completed form to the Fire Medical Qualifications Program office using the number on Part B; questions can go to the program office line printed there.
Who pays
No fee schedule applies; the Forest Service tells employees they are responsible for obtaining, paying for and supplying medical information the Medical Officer requests.

Good to know

  • If the prosthesis needs a battery charger, specify any special power-supply requirements, such as 240V, since fire camps may have limited power.
  • PPE questions apply to anyone issued fire PPE (Nomex): whether the device withstands embers and heat and whether it interferes with or is protected by the clothing.
  • Providers may suggest PPE modifications, such as adding elastic to a cuff so embers cannot enter a sleeve or pant leg, to support accommodation decisions.
  • The form records the driver's license type (CDL, regular or none) and any restriction on driving or working around a chainsaw.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the prosthetic template apply to employees in office-type fire roles?

Part A records the employee's fitness level, Light, Moderate or Arduous, and the general questions apply to all. The PPE section applies to anyone who has possession of fire PPE (Nomex), regardless of level, so the provider should complete it whenever the employee is issued fire clothing.

What mobility question does the prosthetic form ask?

It asks whether the employee, with the prosthesis, can move rapidly on steep terrain and rocky, loose or muddy surfaces, wet leaves and grasses without undue risk of sudden or subtle harm. If the answer is no, the provider specifies the concern for the Medical Officer's review.
