Forest Service Musculoskeletal/Nervous System Form

This four-page Forest Service template covers musculoskeletal conditions and disorders of the central or peripheral nervous system in wildland firefighters. After a screening or OF-178 exam flags such a condition, the treating provider documents diagnosis, procedures, relapses, deficits, pain and hand function, and a Forest Service Medical Officer uses the answers to decide clearance or a waiver.

Who completes it

The employee completes Part A with identifying details and fitness level, which also authorizes release to the program's Medical Officers. The treating provider or relevant specialist completes Part B as applicable and signs with credentials and an office stamp.

Submitting and payment

How to submit it
The provider faxes the form to the Fire Medical Qualifications Program office at the number printed on Part B; there is no online upload for providers.
Who pays
No fee schedule applies; under the program's Request for Information process, the employee is responsible for obtaining and paying for requested medical documentation.

Good to know

  • Part B asks whether there were relapses or recurrences in the last five years and rates the likelihood of a future relapse as low, moderate or high.
  • Describe any neurological deficit by type, severity and distribution, and say whether chronic pain is debilitating or affects function of a body part.
  • The form asks about gross motor skills and grip strength of the hands, and any restriction on driving or operating or working around a chainsaw.
  • It also asks whether the employee can safely move rapidly on steep or uneven terrain or slick surfaces with poor visibility.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What conditions does the musculoskeletal/CNS/PNS template cover?

It is used for musculoskeletal conditions and central or peripheral nervous system conditions in employees seeking Forest Service fire qualifications. The program has a separate template for seizures, syncope, stroke and headache, so the Medical Officer's request indicates which template applies to the employee's diagnosis.

Does the form ask for surgery dates?

Yes. Part B asks for surgeries or procedures related to the diagnosis, with the date of each and the date the employee was released back to full work duties. It also asks whether there are complications such as vascular, gastrointestinal or cardiac problems related to the diagnosis.
