USFS Fire Medical Form: Hearing and Hearing Aids

The Forest Service Fire Medical Qualifications Program uses this hearing template, titled for the years between routine audiograms, when a fire employee has hearing loss or wears hearing aids. Part B asks whether the employee can understand speech with background noise at conversation level, with and without aids, and for arduous duties whether they can do so at 500 to 3000 Hz with aids.

Who completes it

The employee completes Part A (name, date of birth, home unit and light, moderate or arduous fitness level), which also authorizes release to Forest Service Medical Officers. The treating provider (MD, DO, NP, PA or other) completes Part B as applicable and signs it with an office stamp.

Submitting and payment

How to submit it
Fax the completed form to the Fire Medical Qualifications Program office as printed on the form; the program also receives documents through the employee's eMedical account.
Who pays
When a Medical Officer requests more medical information, the employee must obtain, pay for and supply it under 5 CFR 339.304(c).

Good to know

  • Answer the block that matches the employee: no hearing aid used, or hearing aid used with the ear marked as right, left or both.
  • For each no answer, the form asks for the recommended course of action the employee should take.
  • Unlike the General Medical Provider Template, the hearing form has no question on restrictions for 16-hour fire shifts; it centers on speech understanding by duty level.
  • Arduous duty asks specifically about understanding speech with background noise at 500 Hz to 3000 Hz in the aided ear or ears.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

What does the Forest Service hearing aid form ask?

For light or moderate duties, whether the employee can understand speech with background noise at conversation level, with or without hearing aids. For arduous duties with hearing aids, whether they understand speech with background noise at 500 to 3000 Hz. A no answer needs a recommended course of action.

Who completes the Forest Service hearing template?

The employee fills in Part A with name, date of birth, home unit and fitness level, authorizing release to Forest Service Medical Officers. The treating provider completes Part B and signs with credentials (DO, MD, NP, PA or other), address and office stamp.

Can a wildland firefighter wear hearing aids?

The template is built for employees who use hearing aids, asking which ears are aided and how well speech is understood with them. A Forest Service Medical Officer, not the provider, decides qualification and any waiver, and waiver mitigations may include yearly updates.
