OSHA Lead Standard Written Medical Opinion

Under OSHA's general industry lead standard, each lead medical exam or consultation ends with a written medical opinion that the employer must obtain. It states whether the employee has a detected medical condition that raises risk from lead, recommends protective measures and respirator limits, and reports blood lead results. These opinions drive medical removal protection decisions.

Who completes it

The examining or consulting physician writes the whole opinion; if the employee uses multiple physician review, a second or third physician may also issue findings.

Submitting and payment

How to submit it
The physician furnishes the opinion to the employer, who must provide the employee with a copy.
Who pays
The employer pays for lead medical surveillance, including the exam and written opinion.

Good to know

  • Content set by 1910.1025(j)(3)(v): detected conditions, recommended special protective measures, respirator limits including whether a powered air-purifying respirator suits the employee, and blood lead results.
  • The physician must not reveal findings unrelated to lead exposure to the employer, but must advise the employee of any condition needing further care.
  • Removal is required at two blood lead results of 60 µg/100 g or more, or an average of 50 or more; return needs two results below 40.
  • Medical removal protection keeps earnings and seniority for up to 18 months per removal. Construction lead work follows 29 CFR 1926.62.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When must a lead medical exam be offered?

Annually for employees with a blood lead level of 40 µg/100 g or more in the past 12 months, before first assignment to areas at or above the action level, and as soon as possible when an employee reports lead-related symptoms, asks for advice on having a healthy child, or has trouble breathing during respirator use.

What is multiple physician review under the lead standard?

It lets the employee designate a second physician to review the findings of the initial exam. If the two physicians disagree and cannot resolve it, a third physician is designated, and the employer acts on that physician's findings unless the employer and employee agree on an outcome consistent with at least one of the three.
