MSHA 5000-3: Mine Rescue Physical Qualification

Underground mine operators must keep at least two mine rescue teams under 30 CFR Part 49, and each team member needs a physician's certificate of physical fitness for strenuous rescue and recovery work. MSHA Form 5000-3 is that certificate, completed after the member's examination and renewed every year.

Who completes it

The examining physician completes and signs the form for each member of a mine rescue team; it is a physician's certification of physical qualification rather than a patient questionnaire.

Submitting and payment

How to submit it
The completed form stays with the mine operator, on file at the mine rescue station for one year. It is not mailed to MSHA, and the form page warns not to send it to the burden-statement address.

Good to know

  • Each mine rescue team member must be examined by a physician every year, as required by 30 CFR 49.7.
  • The first examination must be completed within 60 days before the member's scheduled initial training.
  • MSHA says the form should be printed and filled in by hand; the PDF is a dynamic form that may only show a 'Please wait' message outside Adobe Reader.
  • The information collection for this certificate carries OMB control number 1219-0144, which expires on 06/30/2029.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
