DS-6546: State Dept Reasonable Accommodation Form

State Department employees requesting a reasonable accommodation give this questionnaire, with a Work Requirements Statement, to their medical provider. The provider describes the condition, the work activities it limits and any accommodation needed, and Department medical professionals and the Disability and Reasonable Accommodation Division (HR/OAA/DRAD) review it to decide the request.

Who completes it

The employee signs the release, adds contact details and writes a separate statement of the limitations and accommodation requested. The medical provider completes the 'For the Medical Provider' pages (3 to 5) and the Medical Provider Information block with signature or electronic certification checkbox.

Submitting and payment

How to submit it
The employee uploads the employee statement and signed medical documentation to the HR Now Employee Portal; all requests start with HR/OAA/DRAD. Questions about the medical submission go to MED Domestic Programs.

Good to know

  • Describe the nature, severity and duration of the condition with objective findings, then which work activities it limits and to what extent.
  • If an accommodation is needed, specify it, explain how it lets the employee work safely, and estimate how long or how often it will be needed and how reassessment will happen.
  • If the provider believes no change is warranted, the form asks for the words 'No work change necessary.' The employee may be offered an alternative effective accommodation.
  • Under GINA, do not include genetic information, including family medical history. The current edition is dated 03-2026.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I answer DS-6546 on letterhead instead of the form?

Yes. The form says the provider may respond on official letterhead with name, address and telephone number, as long as the response covers the same questions: the condition, the limitations, review of the Work Requirements Statement, the accommodation and its expected duration. The provider still signs and dates the response.

Who reads the DS-6546 medical information?

State Department medical professionals and select staff from HR/OAA/DRAD who routinely review such information. The Privacy Act statement says information collected on the form will not be shared outside the Department of State, and that the review follows the Rehabilitation Act and the Americans with Disabilities Act as amended.

What is the Work Requirements Statement?

It describes the employee's job requirements. The employee submits it to the provider with DS-6546 so recommendations are made with full knowledge of the work. The provider reviews it before specifying any accommodation and can list it in the attachments line of the provider information block.
