CDC/NIOSH 2.8 Chest Radiograph Classification

NIOSH's Coal Workers' Health Surveillance Program uses form 2.8 to record each ILO classification of a coal miner's surveillance chest radiograph, under the Federal Mine Safety and Health Act of 1977 and 42 CFR Part 37. The reader grades image quality, classifies small and large opacities and pleural abnormalities, records other symbols and findings, and states whether the miner should see a personal physician.

Who completes it

A NIOSH-certified B Reader, or an A reader at an approved facility, completes the whole form for a single radiograph and signs it, leaving the reader ID blank if not a NIOSH A or B Reader.

Submitting and payment

How to submit it
Submit the completed form electronically to the Coal Workers' Health Surveillance Program by email, as the NIOSH CWHSP forms page directs.
Who pays
NIOSH pays approved facilities and readers for radiographs taken under the coal program.

Good to know

  • The examinee's full Social Security number is optional, but the last four digits are required.
  • The form carries OMB control number 0920-0020 and is marked for official government use only; outside the coal program, readers use the general purpose classification form.
  • Section 4B symbols are obligatory when other abnormalities are present; the 4C checklist is optional and intended to reduce handwritten comments.
  • Related program forms include CDC/NIOSH 2.9 Miner Identification, 2.11 Radiographic Facility Certification and the B Reader Scheduling Form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete CDC/NIOSH form 2.8?

B Readers who interpret chest radiographs for the Coal Workers' Health Surveillance Program, and A readers at approved facilities. The reader marks the type of reading (A, B or F), enters a NIOSH reader ID, initials, reading date and signature, and leaves the ID blank if not a NIOSH A or B Reader.

How is the 2.8 chest radiograph classification submitted?

Electronically, by email to the Coal Workers' Health Surveillance Program, according to the NIOSH CWHSP forms page. The burden statement on page 2 notes that completed forms should not be sent to the CDC paperwork office address printed there.

Can I use form 2.8 for asbestos or silica surveillance?

No. Form 2.8 is for coal miner surveillance radiographs under the CWHSP and is marked for official government use only. For other ILO classifications, such as asbestos or silica medical surveillance, NIOSH offers a General Purpose Chest Radiograph Classification Form, which is not submitted to NIOSH.
