HOPE v1.03: Hospice Outcomes and Patient Evaluation

Hospices complete this standardized patient assessment at admission, at HOPE Update Visits and at discharge for the Hospice Quality Reporting Program. It replaced the Hospice Item Set for data collected from October 1, 2025. CMS uses the data for public reporting of two HOPE-based quality measures, and hospices use it to inform care planning and practice improvement.

Who completes it

Hospice clinicians, mainly the registered nurse with some items from other disciplines, collect the items during visits. There is no patient or family section to complete.

Submitting and payment

How to submit it
Hospices record HOPE in their EHR or vendor software and transmit it electronically to CMS through iQIES. The PDF is a reference item set, not a form to mail.

Good to know

  • Hospices submit up to two HOPE Update Visits at set timeframes, depending on length of stay, covering the first 30 days after the patient elects hospice.
  • HOPE was finalized in the FY 2025 Hospice Wage Index Final Rule. CMS kept key Hospice Item Set items and still uses them to inform the comprehensive assessment at admission.
  • Use Guidance Manual v1.03 with the v1.03 item sets. CMS posts separate Admission, Update Visit and Discharge PDFs and a change table from v1.02.
  • The All Items set carries OMB 0938-1153, expiration 03/31/2028, and CMS says HOPE data may inform future payment refinements.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When did HOPE replace the Hospice Item Set?
