OASIS-E2 Home Health Assessment (All Items)

Home health agencies complete OASIS as part of the comprehensive assessment of Medicare and Medicaid patients at start of care, resumption of care, follow-up, transfer and discharge. The data set case mix under the Patient-Driven Groupings Model (PDGM) and feed home health quality reporting. OASIS-E2 replaced OASIS-E1 on April 1, 2026.

Who completes it

The registered nurse or therapist (physical therapist, speech-language pathologist, or occupational therapist where permitted) who performs the assessment visit completes the items. The patient does not fill in any part.

Submitting and payment

How to submit it
The agency records OASIS in its EHR or vendor software and transmits it to CMS through iQIES within 30 days of the assessment.
Who pays
OASIS responses set the case-mix grouping that determines Medicare home health payment under PDGM.

Good to know

  • CMS posts an All Items PDF plus separate time-point instruments for start of care, resumption of care, follow-up, transfer and discharge in a ZIP file.
  • Final instruments carry OMB 0938-1279, expiration 12/31/2028. CMS's change table lists the differences between OASIS-E1 and OASIS-E2.
  • OASIS is collected alongside the home health plan of care (CMS-485), and the OASIS Guidance Manual explains how to code each item.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When did OASIS-E2 take effect?

OASIS-E2 took effect April 1, 2026, succeeding OASIS-E1. CMS posts the final All Items PDF, time-point versions in a ZIP file, and a change table comparing E1 and E2 on its OASIS data sets page.

Who can complete an OASIS assessment?

The clinician performing the assessment visit: a registered nurse, or a physical therapist, speech-language pathologist or occupational therapist where permitted. The agency then transmits the data to CMS iQIES within 30 days of the assessment.

Is OASIS used for home health payment?

Yes. OASIS responses feed case-mix classification under the Patient-Driven Groupings Model, which sets Medicare home health payment. The same OASIS data, collected at each time point from start of care to discharge, also feed home health quality reporting.
