CMS-2746: ESRD Death Notification Form

When a patient with end-stage renal disease dies, the provider last responsible for their kidney care reports the death to the ESRD Medical Information System on this form. It records the date and place of death, treatment modality, coded primary and secondary causes, whether dialysis was discontinued and why, transplant history, and whether the patient was receiving palliative or hospice care.

Who completes it

The dialysis facility last responsible for the patient's maintenance or home dialysis completes it; for a transplant patient, the transplant center does. Item 18 names the attending physician, but the form has no physician signature.

Submitting and payment

How to submit it
Complete it in the CMS ESRD Quality Reporting System (EQRS) within 2 weeks of the date of death. If the electronic system cannot be used, send a hard copy to the regional ESRD Network.

Good to know

  • Code 104, withdrawal from dialysis or uremia, may not be reported as the primary cause; choose a primary cause from the list, using Other (98) with details if needed.
  • Do not report the same code as primary and secondary cause. Up to four secondary causes may be listed, or mark No secondary.
  • Code 82 applies only when malignant disease was diagnosed after immunosuppressive therapy began. In-transit and dead-on-arrival deaths are recorded under Other place of death.
  • Current edition CMS-2746-U2 (06/2025), OMB 0938-0048, expiration 03/31/2027. It pairs with CMS-2728, the medical evidence report filed when ESRD treatment starts.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
