South Dakota PA-106: Genetic Testing PA Request

South Dakota Medicaid requires prior authorization for genetic testing, and the ordering provider requests it on form PA-106. The form must be sent with medical records supporting medical necessity. Its second page asks the clinician to explain how a positive or negative result would change the recipient's plan of care, and to estimate how common the suspected condition is and how likely the recipient is to have it.

Who completes it

The ordering provider completes the form, including the facility or clinic, anticipated date of service, diagnosis and procedure codes, the referring and servicing providers, and the clinical questions on page 2.

Submitting and payment

How to submit it
Email the form with records to the Division's Medicaid prior authorization mailbox, which the Division says takes all requests until further notice.

Good to know

  • Be specific about the effect of results: medications to add or stop, therapies or procedures that follow, or further testing not already done.
  • Attach results of all diagnostic testing, such as labs, biopsies, pathology reports and imaging, and list current therapies and medications.
  • For a school-age child, say whether there is a care plan or IEP and include a copy if there is.
  • BRCA testing uses its own form, PA-107, rather than the general genetic testing request.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does South Dakota Medicaid ask about the genetic test's value?

The form asks how the outcome will affect the recipient's plan of care if results are positive or negative, with specific medications, therapies or further screening. It also asks for the prevalence of the suspected condition in the population and the recipient's likelihood of having it, both as percentages.

How long does a South Dakota genetic testing PA take?

The Division of Medical Services has 30 days to make a prior authorization determination and says most take around two weeks. Prior authorization applies only to elective services, and the determination notice is sent to the point of contact listed on the form.
