South Dakota Medicaid PA-101 DME and Nutrition PA

South Dakota Medicaid's PA-101 requests prior authorization of durable medical equipment and medical nutrition products. Clinicians meet it when ordering equipment on the Division of Medical Services' list, such as continuous glucose monitors, wound vacs, speech-generating devices or specialty mobility devices, or enteral and parenteral nutrition. The supplier submits it with the prescriber's order and medical records supporting the request.

Who completes it

The DME or nutrition provider completes every field, using the prescribing practitioner's diagnosis codes, order and records; the prescriber appears as the referring provider. All fields are required.

Submitting and payment

How to submit it
Email it with supporting medical records to the Division's Medicaid prior authorization mailbox, which the Division says handles all requests until further notice.
Who pays
For equipment, the form asks for the manufacturer, serial number and both the purchase and rental price.

Good to know

  • Nutrition requests must answer whether the product is the person's sole source of nutrition and whether they live at home, and name the nutrition prescribed.
  • Equipment requests describe the item and its function and should include identifying material such as brochures and pictures.
  • Enteral nutrition needs approval over age 21; for children, EPSDT special requests and non-covered items under 21 also go through prior authorization.
  • Incontinence supplies for members on the state's waivers use a separate request, the PA-108 Incontinence Supply form.
