South Dakota Medicaid PA-100 General PA Request

South Dakota Medicaid's PA-100 is the general prior authorization request for elective services on the Division of Medical Services' prior authorization list, and the Division also uses it for certain surgical and other outpatient services. A clinician meets it when scheduling a listed service that has no dedicated form. It must be sent with medical records supporting the service, and the determination goes to the named point of contact.

Who completes it

The requesting or rendering provider completes the form, including the explanation of problem and prognosis, referring and servicing provider NPIs and taxonomy codes, and a point of contact. The point of contact is the person completing the request.

Submitting and payment

How to submit it
The Division asks that all prior authorization requests be emailed to its Medicaid prior authorization mailbox until further notice, with supporting medical records attached.
Who pays
Retro authorization can be requested after the service when care was suspected to be urgent or emergent at the time, but it is billed as elective.

Good to know

  • Only the elective services on the Division's list need prior authorization; urgent and emergent care is exempt.
  • The Division has 30 days to decide and says decisions usually take about two weeks.
  • If more than one service is expected, include a schedule of planned care for the next year to reduce the number of authorizations needed.
  • South Dakota Medicaid is fee-for-service, with no managed care organizations, so requests go to the Division of Medical Services rather than to a health plan.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
