Illinois HFS 3701H: Seating/Mobility Evaluation

Illinois Medicaid uses the HFS 3701H as the letter of medical necessity for power mobility devices and custom wheelchairs. A physiatrist or physical or occupational therapist, working with a mobility device specialist, documents medical history, home, function, posture, measurements and an equipment trial, then recommends the lowest appropriate equipment. The supplier attaches it to the HFS 1409 prior approval request.

Who completes it

A physiatrist or PT or OT completes the evaluation with the mobility device specialist. The patient or caregiver, therapist and supplier representative sign, and the physician signs last to agree with the recommendations and order the equipment.

Submitting and payment

How to submit it
The supplier named on page 1 attaches the completed evaluation to its HFS 1409 prior approval request to Illinois HFS.
Who pays
The form sets no fee for the physician; it supports the supplier's prior approval for the mobility device.

Good to know

  • Recommendations must name the lowest appropriate group, and the evaluator explains why other equipment was unsuccessful.
  • Equipment trials must last long enough to show independence, especially for a patient whose mobility was previously dependent.
  • The therapist discloses any affiliation with the DME provider, the equipment manufacturer or the patient's long-term care facility.
  • Narrative documentation must be legible. Related forms are HFS 3701K instructions, HFS 3867 Motorized Wheelchair Evaluation and HFS 3701L for standard manual wheelchairs.
