Illinois HFS 3082 Drug Prior Authorization Request

Illinois Medicaid fee-for-service uses HFS 3082 to request prior approval of a drug or an override of a pharmacy limit. Prescribers meet it when a medication needs prior approval or a claim hits an age, sex, quantity, daily dose or brand limit. The form collects the drug, diagnosis, clinical justification and prior medication failures, and is faxed to the HFS pharmacy prior authorization unit for review.

Who completes it

The prescriber or their office completes the whole form; the patient, prescriber and contact-person sections are marked required, and pharmacy details are added when known. The provider signs and dates it.

Submitting and payment

How to submit it
Fax the completed form to the HFS pharmacy prior authorization unit at the number printed on it; the form also lists the prior authorization hotline. It is an Adobe XFA form, so download it and open it in Adobe Reader or Acrobat to fill it in.

Good to know

  • Check a box when asking to override a specific limit: age, sex, brand name, the three-brand limit, maximum or minimum quantity, daily dose, or an emergency 72-hour supply.
  • List every medication previously tried for the indication with the reason it failed, such as side effects or intolerance, and note any current drugs that will stop if this one is approved.
  • The request needs the patient's nine-digit HFS ID number, an ICD-10 code or diagnosis, and a clinical justification for this drug over one that needs no prior authorization.
  • The current revision is HFS 3082 (R-4-16). HFS also publishes HFS 1409X, Pharmacy Prior Authorization Request, and HFS 3082A, a Refill Too Soon Prior Approval Worksheet.
